What is ACL Reconstruction?

ACL (anterior cruciate ligament) reconstruction is a surgical procedure to replace a torn ACL — one of the major stabilising ligaments in the knee. The ACL is most commonly injured during sports involving sudden stops, changes of direction, or pivoting movements.

ACL reconstruction is one of the most frequently performed orthopaedic procedures, and outcomes in Bristol are excellent with the right specialist.

Symptoms of an ACL Tear

  • A sudden “pop” or giving way sensation in the knee
  • Rapid swelling within hours of the injury
  • Significant pain and difficulty bearing weight
  • A feeling of instability, particularly on twisting or turning
  • Reduced range of motion in the knee

Diagnosis

Your knee surgeon will carry out a thorough clinical examination, including specific tests for ligament instability. An MRI scan is the gold standard investigation to confirm an ACL tear and identify any associated injuries to the meniscus or other ligaments.

Treatment Options

Non-Surgical Management

Not every ACL tear requires surgery. In some patients — particularly those with lower physical demands — a structured physiotherapy rehabilitation programme may restore sufficient knee stability. Your surgeon will discuss whether non-operative management is appropriate for your individual case.

ACL Reconstruction Surgery

For patients who wish to return to pivoting sports or who experience ongoing instability, ACL reconstruction is recommended. The procedure involves replacing the torn ligament with a graft, typically taken from your own hamstring tendons or the patella tendon.

Surgery is performed arthroscopically (keyhole) and usually takes around one hour. Most patients go home the same day.

What to Expect After Surgery

  • Week 1–2: Rest, ice, elevation. Commence gentle range-of-motion exercises with physiotherapy
  • Week 2–6: Progressive weight bearing and strengthening
  • Month 3–6: Return to straight-line running and sport-specific rehabilitation
  • Month 9–12: Gradual return to competitive sport, guided by functional testing
